Evidence comes first, then intervention, context, and accountability. A letter on the right is not a verdict on you. It points to a safety mechanism that was easy to miss in that scenario.

Instead of reading an AI answer for longer, put these four questions into the decision record or meeting agenda. They make the human part of the workflow visible.
